These conditions come directly from the Land Transport Act B.E. 2522 (1979). The doctor checks "does not have" (ไม่เป็น) or "has" (เป็น) for each condition.

- The clinic or hospital must be properly licensed to operate in Thailand. This is evidenced by the clinic's operating license (usually displayed in a frame on the wall) and the official clinic stamp on the certificate.
- The doctor must have physically examined you. A certificate issued without an examination (for example, just based on paperwork) would be fraudulent and could result in legal issues.
In practice, any clinic or hospital you can find with a proper storefront and visible operating license will meet these requirements. Unlicensed clinics, roadside medical stations, and pharmacies are not acceptable.
Validity Period: The 30-Day Rule
The medical certificate for a Thai driving license is valid for 30 days from the date of issuance. This means you must submit the certificate to the DLT within 30 days of the examination date printed on the form. If the certificate is older than 30 days, the DLT will reject it and you will need to obtain a new one.
